The top of a radiant or induction cooktop is a single sheet of glass-ceramic resting in a frame over the counter cutout, with the elements or coils directly beneath it and the wiring below those. A crack breaks the seal that keeps spills away from all of that, and a cracked sheet can split further as it heats. Glass is never patched; it is replaced as a part matched to the model, and on many cooktops it comes as a top assembly with its frame. Some edge chips start at the counter itself: an opening cut too tight or a top forced into place can stress the glass at its corners.

Can a cracked glass cooktop be repaired instead of replaced?
No. Sealants and patch kits cannot restore the strength or the seal of glass-ceramic, and a patched top can still split under heat. The sheet, or the whole top assembly, is replaced.
Does a small chip on the rim matter as much as a crack across a zone?
An edge chip that stays at the rim carries less risk, but it still breaks the seal and can grow into a crack. Keep it dry, stop cooking on it, and have it judged in person.
Does the cooktop have to come out of the counter to change the glass?
Often, at least partly, because the glass sits in a frame over the cutout. Power is disconnected first, the top is lifted, and everything is reconnected afterward.
Could the countertop itself have caused the crack?
Occasionally. A cutout that is too tight lets the frame squeeze the glass as it heats. A crack that begins at a corner with no sign of impact is the usual clue.
